SCOPE:
- Consultant will assess your current state in Medicare Population Health Management
- Consultant will outline the benefits in Quality Reporting that the chronic care management provides
- Consultant will outline how this product produces a new revenue stream
DELIVERABLES/OUTPUTS:
- Consultant will introduce your team to the product partners and assist with setting up a demo
TIMING:
- Upon engagement, assessment of current state can occur within a one hour zoom or in-person meeting
- Can introduce you to the product representative within 48 hours of agreement to move forward
ASSUMPTIONS:
- Client will provide current in-depth Medicare direct process.
- Cline will provide relevant source materials
- Additional items outside of this scope of service is an additional engagement and can include a strategic plan, operational plan or cultural / organizational development
